The Fund will be conducting annual open enrollment period from October 2nd, 2023 to November 30th, 2023.
If you are currently enrolled and wish to continue the same coverage, no action is required.
The deadline to complete the enrollment is November 30th, 2023.
If you are enrolling new dependents, you will be required to submit copies of the following supporting documents:
• Social Security Card
• Birth Certificate
• Legal Adoption documents
• Court documents declaring dependency
These documents must be submitted to the Fund’s Administrative Manager, NEBA, or your new dependents will not be added to the plan. This information can be provided by uploading your documents when you are completing your application, via secure portal upload –https://www.nebainc.com/1625upload/ or fax at (954) 266-2079.
